Every employee that is covered by Appendix C Section 1910.134 is required to complete for review, an OSHA Respirator Medical Evaluation Questionnaire. This form is to be completed by the covered employee and reviewed by a Health Care Professional (HCP). Mid-State can provide both the forms and the HCP review.

When certain criteria is met, the employee covered by Appendix C Section 1910.134, is required to have a Health Care Professional perform a physical examination. Mid-State schedules times convenient for both the employer and employee to complete these examinations.

Mid-State can provide individual or large group, quantitative and qualitative Respiratory Fit Testing.

| Mid-State can provide pulmonary function studies as part of the physical examination or group screening of all high risk employees. Each study is interpreted by our Medical Director. All test results are maintained in a confidential medical files according to policy. |
